Other Pay Items

401K: 50% match on the first 5% contributed.  Match is 0% if less than 5% is contributed.
Medical: The company pays 80% of the benefits.  The employee's monthly portion of the medical insurance is $67 (single) and $205 (family).  Dental and vision are separate.  Dental is $6 or $15 and Vision is $6 and $13. Company paid life insurance offered for 1x annual salary and this caps at $50K.
